This final course in the series focuses on comprehensive mastery of collaborative management of adult and geriatric populations with the highest level of complexity of acute and chronic health conditions within the acute care setting. Emphasis is placed on the critical synthesis of multidimensional data and appraisal of evidence-based guidelines and research to lead diagnostic reasoning and prioritize care in fast-paced or specialized acute care environments. Students will design and advocate for therapeutic regimens tailored for diverse and underserved populations, with a specific focus on palliative care, end-of-life transitions, and frailty syndromes. Students will demonstrate mastery of pharmacologic and non-pharmacologic strategies while modeling systems-level leadership along with interprofessional collaboration and communication. The course culminates in the refinement of skills necessary for the safe, high-quality transition of patients from acute stabilization back into the community or long-term care settings.
