This is the final clinical course in a series which provides extensive opportunities for collaborative management for adult and geriatric populations with the highest level of multisystem complexity. Students will demonstrate the critical synthesis of multidimensional data to lead diagnostic reasoning and prioritize care within a variety of acute care environments. Through completion of 300 clinical hours with preceptors in diverse acute care settings, students will propose and advocate for therapeutic regimens that address the unique needs of diverse and underserved populations, with a focus on frailty syndromes, palliative care, and end-of-life transitions. Emphasis is placed on the expert application of pharmacologic and non-pharmacologic strategies while modeling systems-level leadership and interprofessional collaboration and communication. The course culminates in the mastery of skills necessary for the safe, high-quality transition of complex patients across the illness-wellness continuum.
